Ischemic stroke is a leading cause of death and disability, with stroke severity being a key determinant of patient clinical outcome. HbA1c levels reflect long-term glycemic control and are thought to play a role in aggravating neurological damage in stroke. This study aimed to analyze the relationship between HbA1c levels and ischemic stroke severity. This was an observational, analytical, cross-sectional study. The sample consisted of 100 ischemic stroke patients who met the inclusion criteria. HbA1c levels were obtained from laboratory tests and classified as normal (<5.7%), prediabetes (5.7–6.4%), and high (≥6.5%). Stroke severity was assessed using the National Institutes of Health Stroke Scale (NIHSS) and categorized as mild, moderate, and severe. The correlation analysis was performed using the Spearman Rank test with a significance level of 0.05. Most respondents had high HbA1c levels (50.0%). The highest stroke severity was in the moderate category (41.0%). In the high HbA1c group, the proportion of severe strokes reached 38.0%, higher than the normal HbA1c and prediabetes groups. Spearman Rank test results showed a significant association between HbA1c levels and ischemic stroke severity (r = 0.462; p = 0.001). There was a moderate positive association between HbA1c levels and ischemic stroke severity. The higher the HbA1c level, the more severe the stroke.
